1. Download and print the appropriate registration form.
   All forms are in PDF Format.

• Upper School Students (grades 5-8)
• Registration Form with Alternate Class Choice Form



2. Indicate class choices, including Before- and After-School Activities, if needed.

3. Register in one of the following three ways.

  at the SummerGATE Office

NOTE: Please be aware that we cannot process your application without payment. Be sure to include your credit card authorization or a check with your registration info. Whenever possible, please send a check, as the credit card company charges us a substantial fee. We prefer to have your tuition directly serve your child.
